Coordinated Veteran Care Program (CVC)

A CVC is a comprehensive plan that is tailored to meet the unique healthcare needs of veterans. It is designed to provide proactive care coordination for Veteran Gold Card holders with chronic health conditions, and Veteran White Card holders with chronic DVA accepted mental health conditions.
