Your GP is the centre of gravity throughout your healthcare journey, coordinating your access to Allied Health professionals and Specialists, both in-house and across our network. They understand your service history, your unique healthcare needs, and manage your complete healthcare plan with your entire treatment team.
All our GPs are experts in service-aware healthcare, including the medical conditions and lifestyle factors that commonly impact Veterans. This includes elements of your unique service history, ADF culture, and your unique medical needs.
The Coordinated Veteran Care program was developed by the DVA for Gold Card holders and White Card Holders with accepted chronic mental health conditions. CVC plans are coordinated, proactive treatment plans that involve input from your GP, and can include other Allied Health and Specialist professionals, depending on your healthcare needs.
Total Veteran Healthcare can provide your CVC plan in person at our medical centre’s, or conveniently in your home with our nation-wide in-home clinical team.

Health conditions coordinated through your GP, or via a CVC plan, can include:
