Coordinated Veteran Care Program (CVC)

The CVC Program is run by the Department of Veterans Affairs to support eligible Veterans with chronic conditions. It is designed to provide proactive care coordination for Veteran Gold Card holders with chronic health conditions, and Veteran White Card holders with chronic DVA accepted mental health conditions.

The CVC Program is designed to provide Veterans with bespoke, proactive healthcare that reduces the risk of unplanned hospital stays, and involves the input of your entire clinical team, involving the input of multiple clinical specialists, to provide multidisciplinary healthcare.
